Care Manager RN Per Diem Day

Care Manager RN at Petaluma Valley Hospital in Petaluma, CA. This position is Per Diem and will work 8 hour, Day shifts. Utilizing a collaborative process, the Case Manager will establish a continuum of care plan for a caseload of patients. The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients. The Case Manager will act as a liaison to facilitate a smooth transition for the patient from acute to the next level of care.
