Care Manager MSW Per Diem Days

Care Manager MSW Providence Mission Hospital in Mission Viejo, CA. Positions available are Per Diem and will work 8-hour Day shifts Care Management is a collaborative practice model including the patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The care management process encompasses excellent communication, both verbal and written, and facilitates care along a continuum through effective resource coordination and addressing the psych social needs of the patient.
