Job Description

SCOPE: Responsible coordinating the intake process of new patients for supportive care services such as nutrition assistance, medical equipment/supplies and transportation assistance.

QUALIFICATIONS

• High school diploma required
• Some college preferred
• Minimum five (5) years’ social services experience

SPECIALIZED KNOWLEDGE

• Experience in inventory control
