About The Role

Executes and coordinates the day-to-day operation of Almoosa’s population health programs. Working under the
PHM Manager, the Officer turns program design into action — producing patient and care-gap lists, coordinating
outreach and scheduling with Case Management and clinics, tracking program activity, and maintaining the
operational data that underpins PHM measurement. This is an execution-and-coordination role: the ‘doing’ layer
that keeps PHM programs running and measurable.
