About The Role

Owns the clinical and programmatic core of Almoosa’s population health function. Designs PHM programs, defines what
constitutes a care gap, scopes value-based care pilots, and engages clinical specialties across the Group to embed population
health into clinical practice. This is a clinical-programmatic leadership role — not a data analytics role and not a case
management role. The role partners closely with ADA on the data and dashboards each program needs, and with Case
Management on acting upon the care gaps PHM programs surface.
