Case Manager – Veterans Homelessness Support

SCHADS Level 4 | Full-Time | Maximum-Term (2 Years) | Townsville

As a Case Manager, you'll provide person-centred, trauma-informed support that empowers individuals to secure safe housing, strengthen community connections and achieve long-term stability. Every day, you'll work alongside people to overcome barriers, build resilience and create positive, lasting change.

Working within our Homelessness Stream, you'll be part of a supportive team committed to delivering high-quality services that reflect The Salvation Army's mission, values and national Model of Care.

What you'll be doing

In this rewarding role, you will

• Build trusted, respectful relationships that promote choice, independence and self-determination.
• Deliver strengths-based, trauma-informed case management tailored to each person's unique goals and circumstances.
• Complete holistic assessments, identifying needs, risks, strengths and barriers to housing stability.
• Develop and implement individualised case plans in partnership with clients.
• Support veterans and their families to access housing, specialist services and community supports through effective advocacy and referrals.
• Monitor progress, review outcomes and adapt support plans as circumstances change.
• Maintain accurate case notes and records using The Salvation Army's information systems.
• Identify and respond to risks while contributing to a safe and supportive service environment.
• Work collaboratively with colleagues and external agencies to deliver coordinated, high-quality support.

• What you'll bring
• You'll be someone who genuinely cares about making a difference and enjoys working collaboratively to achieve positive outcomes.

You'll also have

• A degree in Social Work, Community Services, Welfare or a related discipline, or equivalent experience and demonstrated sector knowledge.
• Experience delivering case management within community services, with homelessness and/or veteran support experience highly regarded.
• Strong assessment, communication and relationship-building skills.
• The ability to manage complex situations using a person-centred, trauma-informed approach.
• A current Queensland Working with Children Check (Blue Card), or the ability to obtain one.
• A current Australian Driver Licence.
