About The Role

APRN / PA-C – Family Health Center Geriatric Program

Concord Hospital Medical Group is seeking an experienced APRN or PA-C to join our Geriatric Program, providing primary care to older adults in their homes and nursing facilities.

This unique 4-day work week offers a blend of independence and collaboration, with two days spent caring for patients in their homes and two days providing care in nursing facility.

What You'll Do

  • Provide primary care to older adults in their homes and nursing facilities
  • Assess, diagnose, and manage acute and chronic medical conditions
  • Order and interpret diagnostic testing
  • Prescribe and manage medications
  • Educate patients and caregivers on disease management, prevention, and safety
  • Partner with physicians, residents, nurses, and interdisciplinary team members to deliver high-quality care
  • Support Family Medicine residents in the care of geriatric patients
  • Document care and maintain compliance with clinical and regulatory standards

Qualifications

  • Master's degree from an accredited APRN or Physician Assistant program
  • Current or eligible New Hampshire APRN or PA-C licensure
  • Current DEA registration
  • Minimum of two years of post-graduate clinical experience in geriatrics, primary care, home-based care, or nursing facility medicine
  • Experience providing care in home-based and/or nursing facility settings strongly preferred
  • Demonstrated interest in caring for older adults with complex medical needs
  • Strong clinical assessment, communication, and interdisciplinary collaboration skills
