About The Role
QUALIFICATIONS
- High School education or equivalent required.
- Completion and maintenance of a State approved Activity Director Course and current State Certification.
- Must relate well to people and have the ability to establish working relationships with all departments.
- Ability to motivate and encourage resident participation in preferred activities.
- Ability to organize, supervise, and teach others.
- Ability to assume responsibility for planning and implementing group and individual activity programs.
- Possess kindness, poise, dignity and non-judgmental attitude toward the elder resident, family members, and the public.
- Maintain CNA certification.
- Must be able to lift a minimum of fifty (50) pounds.
Requirements
RESPONSIBILITIES
- Work with residents, interdisciplinary team, and the community to provide the broadest spectrum of programs of social and recreational activities.
- Assess and document resident needs and interests upon admission.
- Assess de-escalation interventions for residents with disruptive behaviors.
- Provide support to the resident and family members during the initial separation period to maintain the family relationship.
- Develop an activity plan including individual goals for each resident.
- Document attendance at group activities and preferred individual interests undertaken daily.
- Write a progress note, during the look back period, regarding the resident's progress toward their activity goal for initial, quarterly and annual assessments and effectiveness of the plan.
- For residents with disruptive behaviors, identify interventions utilized to de-escalate behaviors, and effectiveness of these interventions in progress notes during the look back period. (Examples include: Talking quietly, going to a quiet location, skyping with a family member, taking a walk, listening to music).
- Complete the Activity MDS section using the information in the look back period to document resident specific information.
- Write an annual resident activity care plan, attend care plan meetings, and update care plans quarterly.
- Schedule and organize daily, weekly, and monthly events which will be posted in advance on the activity calendar and facility website.
- Distribute and post monthly calendar in each resident's room and in a central location.
- Manage external communication through facility Facebook page, local newspaper, newsletter, and facility website postings of philosophy, mission and activities of the facility.
- Develop contact list for local and surrounding organizations (Lions Club, Kiwanis, Key Club, Quilting Group, Counselor at High School).
- Send written notification of scheduled care plan meetings to resident and responsible party at least two (2) weeks prior to care plan meetings.
- Secure supplies, volunteers and photos of activities each month.
- Perform annual evaluation of activities program soliciting feedback from residents, family members, volunteers, and team.
- Other duties as assigned by your Supervisor.
