About the Role

At Plume, we envision a member experience that provides thoughtful, expert, timely, and gender-affirming care that celebrates and enhances the quality of life for every trans person. As a Care Coordinator, you are the patient's first point of contact. You ensure excellent patient care and satisfaction within a fully remote, telehealth environment, reporting directly to our Care Coordinator Manager.

Responsibilities

  • Answer current and prospective patient questions.

  • Triage inquiries to the appropriate medical staff across multiple platforms.
  • -
  • Ensure accurate data flow across platforms.
  • -
  • Troubleshoot patient technical problems within the internal dashboard system.
  • -
  • Help support our members' labs, pharmacy, and other resource needs.
  • -
  • Assist members with vulnerable mental health through safety planning and intervention.

Must-Haves

  • -
  • A strong understanding and appreciation for the trans experience and a desire to increase access to gender-affirming care.
  • -
  • 2+ years in a customer service role with a demonstrated history of excellent customer/patient/member service.
  • -
  • Great attention to detail in addition to data management and data entry skills.
  • -
  • Respectful and empathetic written and verbal communication.
  • -
  • Basic computer skills and comfort with learning new computer systems.
  • -
  • Comfortable interfacing with patients and colleagues within a fully remote environment.

Compensation

  • $60,000 Annual Salary
